MANDATORY SCHOOL FORMS

Each high school player MUST have a PIAA physical form completed in order to practice and play.   THIS IS MANDATORY.   ELANCO also requires player contracts to be completed. 

The following forms are required and need to be completed, signed and returned to coaches by the first day of practice for participation in a high school sport.

• Drug, Alcohol, and Tobacco Contract
• Garden Spot Consent Form
• PIAA Medical History Form
• PIAA Physical Evaluation Form
• 2 Emergency Procedure Cards

The physical for the 2011-2012 school year needs to be completed after June 1, 2011 and is due by February 24, 2012.
