MANDATORY SCHOOL FORMS

Each Middle School and High School Player MUST have a PIAA Physical Form completed in order to Practice and Play in March.   THIS IS MANDATORY.   Here are the links to the forms.   ELANCO also requires player contracts that also must be completed.   All Forms must be completed and given to Jodi by Valentines Day!

The following forms are required and need to be completed, signed and returned to coaches by the first day of practice for participation in a high school sport and middle school sport.

• Drug, Alcohol, and Tobacco Contract
• Garden Spot Consent Form
• PIAA Medical History Form
• PIAA Physical Evaluation Form
• 2 Emergency Procedure Cards

The Physical for the 2009-2010 school year needs to be completed after June 1, 2009.
